The incredible benefits of massage are doubly powerful if taken in regular “doses.” Taking part in this form of regularly scheduled self-care can play a huge part in how healthy you’ll be and how youthful you’ll remain with each passing year. Membership makes it even BETTER & AFFORDABLE!

PAID-IN-FULL MEMBERSHIPS — Members who purchase a Paid In Full Membership have access to all of their membership sessions immediately upon payment in full. These membership sessions can be used anytime as long as the membership is current and active.
HOW IT WORKS:
Massage membership monthly payments are automatically debited from the member’s registered credit/debit card monthly on the selected date indicated by the client. The monthly charges will continue automatically each month for 6 months, or until the termination of the membership.
Your monthly membership entitles you to a single “Any 60 Minute Session” of your choice each month, on or after your scheduled payment date
You may schedule additional 60min massages at the membership rate
You may upgrade to a longer session at an additional charge
You may also share these accrued sessions
If you do not use your session for that month, it will simply roll-over into next month.
If your membership monthly payment is made you will be considered an active member in good standing.
MEMBERSHIP TERMS & CONDITIONS:
A 1-hour session consists of the FULL 60-minute session time.
This membership and services purchased under this membership are non-transferable to any other person or entity.
For purposes of identification and billing, you agree to provide us with current, accurate, complete and updated information including your name, address, telephone number, and applicable payment data. You agree to notify us promptly of any changes.
You have the entire length of your membership to use your membership services as long as your account is not frozen and is in good standing. Any unused services will expire should you elect to cancel your membership or not to renew your membership.
All Memberships are auto-renewable. At the end of the initial term of membership, your dues will automatically renew on a month-to-month basis unless you have elected otherwise on the Membership Agreement.
We reserve the right to revoke membership at any time for misconduct on our premises. You will be responsible for payment in full upon revocation of membership.
We reserve the right to change pricing at any time upon reasonable notice. Should prices increase, you have the right to cancel membership without penalty during the initial period following cancellation guidelines as described below.
We have the right to refuse or discontinue service for any reason.
Pre-paid membership services have no cash value, are non-transferable, and services expire 12 months from the date of purchase according to agreed terms. We reserve the right to terminate or deny re-enrollment for an indeterminate amount of time if you have an unsatisfactory payment history.
You have the right to receive notice in the event we make any change to the terms and conditions of your membership that will vary the amount to be periodically billed to your account specified above. Except as expressly provided herein, we may modify our services or the terms and conditions of this Agreement at any time without notice and such modifications shall be deemed effective immediately upon making such changes.
CANCELING YOUR APPOINTMENT:
You may cancel your appointment without charge if you cancel within 24 hours of your appointment. Cancellations without 24-hour notice may result in a $35.00 charge for your session, as that time has been set aside specifically for you. No shows may result in a full charge for that session.
CANCELLATION OF MEMBERSHIP DURING INITIAL TERM:
You may cancel this agreement during the initial term of your membership upon the following conditions: Your death or permanent physical disability, or if you move more than an hour away. Written notice must be given at least 30 days in advance to avoid additional charges to your account. Contracted membership services will be forfeited at the time of cancellation effective date.
CANCELLATION OF MEMBERSHIP DURING AUTO RENEW TERM:
After the initial 6 months term, you may cancel at any time. All cancellations require 30 days written notice or email and are effective within 10 business days after the 30-day notice period. Payments due prior to the effective date will be charged as scheduled.
If through no fault of ours, your payment account does not contain sufficient funds to complete the transaction, or your payment account or credit card does not otherwise permit the transaction to be executed, you will be charged a $35 insufficient funds fee. We will contact you to update your account with a working payment method.
We appreciate that you’ve chosen Balsam Rose Massage for your massage and bodywork needs. To provide the best service possible to our clients we have implemented the following policies.
Therapeutic massage is intended to enhance relaxation, reduce pain, increase range of motion, improve circulation, and offer a positive experience of touch. The massage therapist does not diagnose illness, disease, or any other physical or mental disorders. As such, the massage therapist does not prescribe medical treatment or pharmaceuticals, nor do they perform spinal manipulations. Massage is not a substitute for medical treatment or diagnoses and it is recommended that you see a physician for any physical ailments that you may have.
None of the information or services provided is intended to act as a substitute for medical advice, nor does it involve the diagnosis, prognosis, or prescription of remedies for the treatment or prevention of any disease or ailment.
- It is the responsibility of the client to inform us of any current medical conditions or any changes in health and provide written permission (if under care) from a physician to the clinic before each session.
- The client will come to the massage therapy appointment clean.
- If any pain or discomfort is experienced during the session, the client will immediately communicate with the therapist so the treatment can be adjusted.
- Both the massage therapist and the client are vulnerable to infections. Therefore, clients and the therapist are asked to reschedule appointments when they are feeling unwell.
- Payment is due in full at the time of service–cash, credit card, Venmo, ZellePay, ApplePay, CashApp
We respectfully ask that you provide us with a 24-hour notice of any schedule changes or cancellation requests. Please understand that when you cancel or miss your appointment without providing a 24-hour notice we are often unable to fill that appointment time. This is an inconvenience to your therapist and also means our other clients miss the chance to receive the services they need. For this reason, you will be charged 50% of the service fee for the first missed session and 100% of the service fee for each session after that. We also reserve the right to require a credit card number to be given to book future appointments so that appropriate fees may be charged if a late cancellation does occur.
We understand that emergencies can arise, and illnesses do occur at inopportune times. If you have a fever, a known infection, or have experienced vomiting or diarrhea within 24 hours prior to your appointment time, we request that you cancel your session. Inclement weather may also result in the need for late cancellations. We will do our best to give advanced notice if we are closing or need to cancel due to bad weather and we ask you to do the same. Please do not risk your own safety trying to make your appointment. Late cancellation due to emergency, illness, or inclement weather will generally not result in any missed session charges, but this is determined on a case-by-case basis.
Late Arrival Policy
We request that you arrive 5-10 minutes prior to your appointment time to allow time to fill out any required paperwork as well as answer any intake questions your therapist may have. We understand that issues can arise that may cause you to be late for your appointment. However, we ask that you call to inform us if this ever occurs so we can do our best to accommodate you. Appointment times are reserved for each client, so oftentimes we cannot exceed that reserved time without making the next client late. For this reason, arriving after your appointment time may result in loss of time from your massage so that your session ends at the scheduled time. Full-service fees will be charged even when sessions are shortened due to late arrival. In return, we will do our best to be on time, and if we are unable to do so we will add time to your session to make up for our late arrival or adjust the service charge accordingly.
Inappropriate Behavior Policy
Massage therapy is for relaxation and therapeutic purposes only. There is absolutely no sexual component to massage whatsoever. Any insinuation, joke, gesture, conversation, or request otherwise will result in immediate termination of your session and a refusal of any and all services in the future. You will be charged the full-service fee regardless of the length of your session. Depending on the behavior exhibited we may also file a report with the local authorities if necessary. Treat your therapist with respect and dignity and you will be treated the same in return.
